Book A Consultation

To be seen by one of our ophthalmologists, you will need a referral from either your optometrist or another doctor (family doctor or another specialist).

They can contact us by phone, fax or via email.

Your optometrist or referring doctor must fill out all the requested information on the provided referral form and send it by fax to one of the numbers below:

The medical professional’s signature and practice number must clearly appear on the form. It must also contain your name, your phone number as well as your date of birth and a valid health insurance number. 

Once we receive the referral form, one of our administrative agents will contact you as soon as possible to offer you an appointment.

What To Bring 

For your consultation you should bring the following items with you:

  • The consultation request form (if not already sent)

  • Valid health insurance card

  • Your glasses 

  • Contact lens case and fluid (we might ask you to take off your contacts for the examination)

  • Any eye drops you are using

  • An updated list of medication from the pharmacy

  • A driver (we might need to dilate your pupils during the examination. If so, you cannot drive afterward)

  • A valid method of payment, if applicable (cash, credit, or debit card)

    • Your consultation is covered by the RAMQ, as well as your treatments. Certain tests that are not covered by the RAMQ can have fees, however, these tests can usually be done at the hospital at no cost. We always do our best to notify you in advance of the potential fees related to your examination.
